چکیده
Introduction. Rapid diagnosis of metformin-associated lactic acidosis (MALA) and initiation continuous renal replacement therapy (CRRT) in diabetic patient successfully corrects a severe metabolic disorder with acute failure. Case outline. A 58-year-old male medical history type 2 diabetes alcohol abuse was admitted to the Emergency Department because vomiting, diarrhea, altered mental status. Initial arterial blood gas analysis revealed (pH: 6.8, PaCO2: 12 mmHg, HCO3: 3.2 mmol/l), but lactate level too high measure. MALA suspected based on progressive past intake metformin. Renal initiated ? veno?venous hemodiafiltration, as result significant improvement clinical status, both pH showing normalization, achieved after finishing CRRT. Conclusion. carries an ominous prognosis. This case suggests early CRRT hemodynamically unstable patients MALA.

Management of Metformin-Associated Lactic Acidosis by Continuous Renal Replacement Therapy
BACKGROUND Metformin-associated lactic acidosis (MALA) is a severe metabolic failure with high related mortality. Although its use is controversial, intermittent hemodialysis is reported to be the most frequently used treatment in conjunction with nonspecific supportive measures. Our aim was to report the evolution and outcome of cases managed by continuous renal replacement therapy (CRRT). M...

Metformin has been used in Europe since the 1970s to treat type 2 diabetes mellitus (T2DM). Usage has increased since the results of the UK Prospective Diabetes Study were published [1]. Metformin is widely endorsed as initial therapy by professional organizations because of low cost, a good safety profile, and potential protection against cardiovascular disease [2]. Therefore, it is now consid...

: Metformin is the most commonly prescribed oral antidiabetic agent. Despite a good safety profile in most patients with diabetes, the risk of metformin-associated lactic acidosis is real if safety guidelines are ignored. Experience with 3 cases of metformin-associated lactic acidosis is reported. Two cases were caused by inappropriate use of metformin in the presence of renal, cardiac and hepa...

A 43-year-old woman was brought to the hospital with severe metabolic acidosis (pH 6.56, bicarbonate 3 mmol/L, and lactate 18.4 mmol/L) and a serum creatinine of 162 μmol/L with a serum potassium of 7.8 mmol/L. A delayed diagnosis of metformin-associated lactic acidosis was made, and she was treated with tris-hydroxymethyl aminomethane (THAM) and renal replacement therapy (RRT). Following a com...
